<p>From a distance, it looks like Tal Broustin is lighting up a cigarette, right in the middle of Arundel Mills, a clear no-no. And he is trying to get others to take drags, too, luring passersby to his kiosk by asking if they are trying to quit smoking.</p>
<p>Up close, it is clear that Broustin is taking puffs not from an actual cigarette, but from a battery-powered gadget designed to look like the real thing. Called an "e-cigarette," or electronic cigarette, it contains no tobacco, gives off no smoke but instead is a nicotine delivery device that gives off heated water vapor. Some companies are pitching e-cigarettes simply as less harmful alternatives to smoking, saying that smokers who can't quit might be better off "vaping" one of their products. Other companies, though, are selling their e-cigarettes as smoking cessation tools, claims that have not been backed up by any science.</p>
<p>Regardless, the relatively new devices - available online, at truck stops and at mall kiosks like the one where Broustin works - are drawing fire, mostly from groups such as the American Cancer Society and the American Lung Association, and from scientists who say they fear the products may pose unknown dangers, even if they're not from the known carcinogens in cigarette smoke.</p>
<p>Some have called on the Food and Drug Administration to ban them immediately. The FDA says e-cigarettes are "unapproved drug-device combinations," and, in the past two months, has detained 17 shipments from China at the border and sent them back. "We don't know its safety profile," said spokeswoman Karen Riley.</p>
<p>Despite the recent FDA actions, the industry says more than 100,000 e-cigarettes have been sold in the United States, and the number grows every day.</p>
<p>"Relatively little is known about how they're used, how much nicotine gets into people, what other chemicals are coming along for the ride," said Dr. Jonathan Samet, director of the Institute for Global Health at the University of Southern California and a former department chairman at the Johns Hopkins School of Public Health in Baltimore. "To make a therapeutic claim, you need to do the proper testing."</p>
<p>One selling point of e-cigarettes is that they can be used where smoking is banned - in bars, in restaurants, at the mall. Samet said he thinks people who use them could get even more nicotine than before because they will be able to "light up" in more places. One consequence of smoking bans, he said, has been that more people have quit smoking.</p>

<p>Jack Leadbeater, chief executive officer of NJOY, a Scottsdale, Ariz., e-cigarette maker, is chairman of the E-Cigarette Association. The industry group includes many of those who sell the products, though it doesn't include Smoking Everywhere, the company whose kiosk is at Arundel Mills and other malls. He says his association's members make no claims that their device will help people quit smoking.</p>
<p>"It's really sold just as an alternative to allow current smokers to get nicotine in a manner that's more palatable," he said. "If this was a form of vodka that didn't cause liver damage, would we be having the same sort of problems?"</p>
<p>A starter e-cigarette kit typically goes for about $100. When the user inhales through the cigarette-like tube, a heating element is activated and it vaporizes a nicotine solution stored in the mouthpiece. A red light will glow on the end, simulating the burning of tobacco. The mouthpiece contains about the same amount of nicotine in a pack of cigarettes and is flavored to taste like tobacco or menthol (though some offer mint, vanilla or other flavors). The nicotine needs to be replenished at an additional charge</p>
<p>William T. Godschall, executive director of Smokefree Pennsylvania, said he finds the debate about e-cigarettes to be counterproductive. He agreed that clinical trials have not been conducted, but he finds it odd that a government that can't seem to regulate cigarettes - which are known to cause cancer - is upset over a much less hazardous product.</p>
<p>"These e-cigarettes are at least 99.9 percent less deadly than cigarettes," he said. "Let's worry about the products that are actually killing people."</p>
<p>Back at the Smoking Everywhere mall kiosk, Broustin is closing in on a sale. He has blown vapor into the air to show James Papanicolas, a 19-year-old mover from Laurel, that it doesn't stink like cigarette smoke. He has explained that his product can actually help reduce nicotine addiction by allowing a smoker to choose less and less concentrated amounts of the drug over time.</p>
<p>Papanicolas' friend, Rose Sanders of Taneytown, isn't sold. The 27-year-old doesn't understand how an e-cigarette could ease her addiction. "It's still going to make you want another cigarette," she said. "If you have this, you're still thinking about smoking."</p>
<p>But Papanicolas planned to buy the e-cigarette. He thinks it will save him money in the long run, since the new filters cost far less than the $6 to $8 he shells out for a pack of Newports.</p>
<p>Will it help him stop smoking? "It might," Papanicolas said. "I know it will help me stop buying cigarettes."</p>

<p>SALT LAKE CITY &mdash; In his zeal to keep Utahns away from the hazards of nicotine, a state lawmaker could be putting thousands of smokers trying to quit in harm's way, a group promoting electronic cigarettes that would be banned by HB71 said Monday.</p>
<p>Rep. Paul Ray, R-Clearfield, would sooner have people using the most dangerous source of nicotine &mdash; cigarettes &mdash; than allow smokers a way to get nicotine through a smokeless device that doesn't come with the thousands of toxic compounds in burning tobacco, said a spokesman for the National Vapors Club (<a href="http://www.vapersclub.com" target="_blank">www.vapersclub.com</a>.)</p>
<p>The battery-powered, reusable e-cigarettes heat a small vile of liquid nicotine enough to vaporize it but not enough to cause it to smoke, said group spokesman Spike Babaian. The bill embodies the widespread but incorrect notion that nicotine and smoking are the same things and pose the same health hazards, he said.</p>
<p>While it is true that people smoke mostly because of nicotine, Babaian said, nicotine generally is a stimulant that acts like caffeine in the bloodstream and that people who inhale it by smoking tobacco die mostly because of the tars and other cancer-causing compounds absorbed from the smoke.</p>

<p>Ray said quibbling over which of the many poisons found in tobacco is more hazardous to human health is an attempt to muddle the issue. That fact is nicotine is a powerful poison that is a very effective pesticide, and the amount of it contained in five cigarettes or half a cigar would potentially kill an adult and definitely be fatal to an infant if were absorbed all at once and no medical care were provided, he said.</p>
<p>Babaian said passage of the bill would take away a life-saving choice for Utah smokers, and it leaves smokers who have already successfully switched to electronic cigarettes in an impossible situation.</p>
<p>"It doesn't take a rocket scientist to see which is the better choice," he said. (The bill) is inhumane, and it certainly isn't in the interest of public health."</p>

<p>The FDA, which conducted research into e-cigarettes, has expressed concerns about their safety, and teams from Greece and New Zealand have also carried out studies into them.</p>
<p>But interpretations of the three reports vary, with the New Zealand study saying e-cigarettes should be recommended because they are safer than tobacco cigarettes, and the Greek study taking a broadly neutral stance.</p>
<p>"The limited information given in these three reports represents all the knowledge we currently have about e-cigarettes," Andreas Flouris and Dimitris Oikonomou, of the Institute of Human Performance and Rehabilitation in Greece, wrote in the British Medical Journal.</p>

			<description><![CDATA[<p>Even in these days of strict indoor clean air laws, you can still legally puff away in movie theaters, restaurants or even on a plane. You just have to use a cigarette that runs on a battery, not tobacco.</p>
<p>Electronic cigarettes &mdash; battery-powered devices that deliver a fine spray of nicotine without any flame or smoke &mdash; have been sold in this country for about three years now. Some people use them as a way to quit smoking real cigarettes. Unlike gum or patches, the devices mimic the sensation of smoking while providing the nicotine rush. Other people use them to get their cigarette fix in places where smoking is not allowed.</p>
<p>The Food and Drug Administration hasn't approved electronic cigarettes as an aid to quit smoking or for anything else. The agency has tried to stop the products from entering the country, but its authority over e-cigarettes is still being hashed out in courts. Meanwhile, the gadgets have developed a devoted fan base. On message boards and blogs, e-cigarette users have loudly and clearly proclaimed their allegiance to the devices.</p>
<p>E-cigarettes &mdash; sold without age restrictions online, in some bars and clubs, and at the occasional mall kiosk &mdash; come with replaceable cartridges containing various levels of nicotine. The regular cartridges for NPRO cigarettes from NJOY, for example, contain 18 milligrams of nicotine, but you can also get a light version with 12 mg., an ultralight version with 6 mg. and even a nicotine-free version. The company says that, for a typical smoker, each cartridge would last about as long as a pack or pack-and-a-half of cigarettes. For comparison, a smoker would get about 20 mg. of nicotine from a single pack of regular cigarettes. Each cartridge also contains water and propylene glycol, a chemical that helps disperse the nicotine. (Propylene glycol also is a key component of fog made by fog machines, should you be curious.)</p>
<p>The cartridges come with a variety of flavors. Users of the No. 7 cigarette from SS Choice, for example, can choose from tobacco, menthol, blueberry, vanilla and other options &mdash; as well as a range of colors. <p>Claims: The FDA does not allow e-cigarette companies to market the devices as aids to quit smoking. But both Ron MacDonald, president of Crown7, and Jonas Joiner, marketing director of SS Choice, say they know of plenty of ex-smokers who used e-cigarettes to wean themselves off the real thing. (A spokesperson for NJOY declined to comment because the company is currently in litigation with the FDA.) Some users manage to give up nicotine completely, but others just switch from one source of nicotine to another, MacDonald says. "A lot of people continue to use our product instead of regular cigarettes, and they feel better," he says.<br />The NJOY website says that an e-cigarette provides "all the pleasure and sensation of smoking without all the health, social and economic problems." The SS Choice site says that its cigarette is more convenient and cost-effective than a regular cigarette and doesn't contain "the thousands of additives and chemicals" found in cigarettes. The Crown7 site says that its cigarettes are "the latest way to get a nicotine fix without most of the harmful effects associated with smoking."</p>
<p>The bottom line: Electronic cigarettes certainly have their appeal in a gadget-centric world, but many questions remain, says Thomas Eissenberg, director of the clinical behavioral pharmacology laboratory at Virginia Commonwealth University in Richmond. For one thing, he says, e-cigarettes have never been studied for smoking cessation, so nobody knows if they are any more or less effective than other nicotine replacement products such as gums or patches.</p>

In a study published in July, Eissenberg and colleagues had 32 smokers take puffs on two brands of e-cigarettes, the NPRO with 18 mg. of nicotine and a model from Crown7 with 16 mg. Each smoker took 20 normal puffs, with a short break after the first 10. The study found that electronic cigarettes did reduce the urge to smoke, but, importantly, neither brand delivered measurable amounts of nicotine to the users' bloodstreams. Eissenberg suspects that the devices are so similar to cigarettes that they can help fill the psychological urge to smoke even if they don't provide much nicotine.<br />"I've been told that you can get nicotine from these things if you really work at it," Eissenberg adds. He believes it's possible that real-life users can get all the nicotine they need by drawing hard, inhaling deeply and taking many puffs. He's considering running a study under more realistic conditions with smokers who are already familiar with e-cigarettes.</p>
<p>In another study published in July, researchers at UC Riverside used a smoking machine to show that it takes more suction to get nicotine from an electronic cigarette than from a regular cigarette.</p>
<p>Eissenberg believes that electronic cigarettes may be worth a try for someone who wants to quit smoking. "Using it for six to eight weeks could be good," he says. But long-term use is another matter, he adds. Although propylene glycol is generally considered to be safe for humans, he warns that nobody knows the effects of deeply inhaling the compound for hours at a time on a daily basis. "I don't know that that's safer than cigarettes," he says.</p>
<p>Adam Leventhal, director of USC's health, emotion and addiction laboratory, says that electronic cigarettes are almost certainly safer than cigarettes. Still, he believes that nicotine patches and gums would probably be better options for anyone looking to quit smoking. Such products are regulated by the FDA, and users can be confident that they're getting the amount of nicotine that they expect. And because e-cigarettes are so much like regular cigarettes, he's concerned that they won't do much to break the actual habit of smoking. "We always tell people who are trying to quit smoking that they should get rid of anything that reminds them of smoking, including ashtrays and cigarettes," he says. "Electronic cigarettes could be a trigger."</p>
<p>Experts and policymakers who criticize electronic cigarettes are missing a great chance to improve public health, says Carl Phillips, a former associate professor of public health at the University of Alberta in Edmonton and creator of tobaccoharmreduction.org, an academic website that promotes alternatives to smoking.</p>
<p>He says the effects of propylene glycol should be studied, but he's confident that e-cigarettes are at least 95% safer than actual cigarettes. "If users are engaging in something that feels the same as smoking but doesn't involve any smoke, that's great," he says.</p>

<p>Tobacco use causes enormous morbidity and mortality because of the high risk of smoking-related diseases and the high prevalence of cigarette smoking. Existing smoking cessation methods only help motivated smokers who are ready to quit, but the vast majority of smokers are pre-contemplators who are neither ready nor willing to attempt to quit. This means that a high proportion of smokers are not adequately served by current strategies for treating tobacco dependence. As cigarettes prematurely kill 50% of long-term users, any additional measure that may reduce death or illness should be given serious consideration. Many addicted smokers are now forced to live and work much of their life in environments in which smoking is prohibited. Most smokers are dependent on nicotine and abstinence from smoking results in tobacco withdrawal and craving, which manifest as clinical symptoms within a few hours of smoking the last cigarette. Craving and withdrawal symptoms can be controlled by supplying nicotine from sources other than cigarettes, such as Nicotine Replacement Therapy (NRT). Clinical studies of short-term abstinence show that all NRT formulations relieve tobacco withdrawal symptoms and craving. Most unaided attempts to decrease health risks by reducing smoking fail because smokers revert to their 'usual' nicotine intake. However, using NRT to reduce smoking allows smokers to reduce their cigarette consumption (and intake of toxic substances in smoke) while maintaining their nicotine dose. Data suggest that smokers who use NRT can significantly reduce the withdrawal symptoms and craving caused by abstaining from cigarettes, and thereby reduce the number of cigarettes/day and maintain these reductions for up to 2 years. The data also indicate that, despite some compensatory smoking behaviour, reduced smoking with NRT results in decreased toxin exposure. In smoking reduction studies, this translated into an improvement in variables that impact on health: cardiovascular risk factors and haemorheology parameters moved towards more healthy (i.e. non-smoker) levels, and pulmonary function improved. The improvements in established cardiovascular risk factors provide objective proof that exposure reduction translates into clinically meaningful health benefits. Furthermore, the known reversibility of many smoking-induced diseases, the mainly linear dose-effect curve and the absence of any indication of threshold effects suggest that additional health benefits may result from smoking reduction. Even more importantly, smoking reduction may move smokers along the behavioural model towards the ultimate goal--stopping smoking. In all three large smoking reduction studies, a number of subjects who were unwilling or unable to stop smoking at baseline were abstinent at 4 months and 1 and 2 years, which clearly supports the concept of smoking reduction as a step towards abstinence. Rather than undermining cessation, smoking reduction appears to increase motivation to quit. The importance of allowing smokers to gradually take control of their smoking was reflected by the increasing point prevalence abstinence rates seen in the long-term studies. When encouraging smoking reduction, it should clearly be emphasised that complete cessation remains the ultimate goal, but smokers in the precontemplation stage need to progress along the behavioural model before becoming receptive to messages about quitting. In conclusion, the evidence presented in this review supports reduced smoking as a legitimate treatment approach that could be pursued by those smokers who are currently unable or unwilling to quit. Sustained smoking reduction can be achieved and maintained with NRT. The corresponding reduction in exposure is associated with tangible health benefits, measured using surrogate markers. Smoking reduction also promotes abstinence in smokers who are unable or unwilling to quit smoking abruptly. NRT is well tolerated for smoking reduction, and nicotine intake does not increase during concomitant use of NRT and smoking.</p>

Furthermore, the known reversibility of many smoking-induced diseases, the mainly linear dose-effect curve and the absence of any indication of threshold effects suggest that additional health benefits may result from smoking reduction. Even more importantly, smoking reduction may move smokers along the behavioural model towards the ultimate goal--stopping smoking. In all three large smoking reduction studies, a number of subjects who were unwilling or unable to stop smoking at baseline were abstinent at 4 months and 1 and 2 years, which clearly supports the concept of smoking reduction as a step towards abstinence. Rather than undermining cessation, smoking reduction appears to increase motivation to quit. The importance of allowing smokers to gradually take control of their smoking was reflected by the increasing point prevalence abstinence rates seen in the long-term studies. When encouraging smoking reduction, it should clearly be emphasised that complete cessation remains the ultimate goal, but smokers in the precontemplation stage need to progress along the behavioural model before becoming receptive to messages about quitting. In conclusion, the evidence presented in this review supports reduced smoking as a legitimate treatment approach that could be pursued by those smokers who are currently unable or unwilling to quit. Sustained smoking reduction can be achieved and maintained with NRT. The corresponding reduction in exposure is associated with tangible health benefits, measured using surrogate markers. Smoking reduction also promotes abstinence in smokers who are unable or unwilling to quit smoking abruptly. NRT is well tolerated for smoking reduction, and nicotine intake does not increase during concomitant use of NRT and smoking.</p>
